Please use the comment section of the form to indicate any additional information pertaining to your order.  This form is utilized to inform the school that you are purchasing a item(s) from the student store. Once the order form has been received, your monthly school statement will reflect your purchase. After payment has been received, you will be notified that you may pick up the item(s) at the front desk of the Middle School or Lower School.
